Modem not connecting to internet - Modems

My USB modem is not connecting to internet from last few days?

It against to work good earlier but since last saturday whenever I tried to tie it said "ERROR 721: REMOTE COMPUTER DID NOT Pity". What is the problem guys?

Additional info:
TATA Indicom USB Modem (CDMA)

OS:
Microsoft XP SP3

If it was working before and you didn't transmute any configuration on your network connection, It seems that the problem is in your ISP network. Referring to the mistaken log, probably the ISP internet server was down. You should ask your ISP about it.
